About this event

The recent public health emergency confirmed that high quality, patient-centered primary care is the essential core of an effective, efficient health system. In our "Resilient Primary Care" webinar series, CQC will support primary care practices to build and strengthen internal capacity through facilitating peer learning and spotlighting successful practices, covering topics relevant to both the foundation of high-performing primary care and health care innovations that will bolster practices as they adapt for the future. 

Please join us for the first session in this series, in which we will highlight successes and challenges for practices using virtual care to provide chronic disease care, and share information on CQC's next technical assistance program, the CalHIVE Network. Front line providers and care teams, IPA leaders and staff are welcome to attend; health plans and other technical assistance partners are encouraged to join, listen and share.  

By the end of the webinar, participants will:

  • Surface opportunities and identify challenges in sustaining virtual chronic disease care during and beyond COVID-19
  • Discover lessons learned shared by organizations experienced in virtual chronic disease care, and how they can be applied to your organization·  
  • Hear how participation in CQC’s CalHIVE Network. can optimize virtual care at your organization
